Hayden Panettiere’s Cause Of Death Revealed — Actress Died At 36 After Sudden Medical Emergency

Hayden Panettiere’s official cause of death has been revealed, more than a month after the actress died unexpectedly at the age of 36.

The Heroes and Nashville star died on Aug. 16 in Greenville, South Carolina, where she had been temporarily staying. Her death was confirmed by her family at the time, with her father, Alan Panettiere, remembering his daughter as “an incredible light and a force of nature.”

Now, the Greenville County Coroner’s Office has released its findings.

Coroner Rules Her Death An Accident

According to the coroner’s statement released Tuesday, Sept. 22, Panettiere’s death was caused by the toxic effects of fentanyl, 4-ANPP, alprazolam, methocarbamol and quetiapine.

The manner of death was ruled an accident.

The coroner said emergency services were called at approximately 1:51 p.m. on Aug. 16 following a report of cardiac arrest. First responders found Panettiere in cardiac arrest and attempted advanced cardiac life support, but resuscitation efforts were unsuccessful. She was pronounced dead at 2:32 p.m.

An autopsy was subsequently performed by a board-certified forensic pathologist.

The coroner’s office said no signs of trauma were discovered that contributed to her death.

A Devastating Loss At Just 36

Panettiere became a familiar face to audiences from a young age, appearing in films including Remember the Titans before gaining international recognition as Claire Bennet on the hit series Heroes.

She later starred as Juliette Barnes on the musical drama Nashville, earning praise for her performances both on screen and through the show’s music.

Her final years were marked by a number of deeply personal struggles, which she spoke about publicly in an effort to challenge misconceptions surrounding addiction, recovery and mental health.

In May 2026, months before her death, Panettiere appeared on Jay Shetty’s On Purpose podcast and discussed her past struggles with alcoholism, substance abuse and postpartum depression.

She also addressed public speculation surrounding her daughter, Kaya, whom she shares with former fiancé Wladimir Klitschko.

Panettiere rejected claims that she had simply “given away” her daughter after Klitschko received full custody in 2018.

“I think a misconception is that I have been, in the past, forced into treatment when, in fact, I have been the one who has sought it out,” she said at the time.

Her Family Is Now Left To Grieve

Panettiere is survived by her daughter, Kaya, who was born in 2014.

Following the actress’ death, her family remembered her not simply as a Hollywood star, but as a daughter and mother whose life had touched many people.

Her father described her as an “incredible light” and a “force of nature,” words that now carry an even deeper weight following the release of the coroner’s findings.

Panettiere’s death had initially prompted an investigation involving the Greenville County Coroner’s Office and Greenville City Police Department. Her body was released to her family on Aug. 18 and transferred to a mortuary that evening.

The official ruling now provides an answer to one of the biggest questions surrounding her sudden death, while her loved ones continue to mourn a woman who died far too young.
